This rider provides the sum assured to the insured in case of:


a) Loss of independent existence, which for the present rider means the total and permanent disability of the insured to perform independently at least three Activities of Daily Living (ADLs) of a common human, which are as follows:

• Bathing 
• Dressing and undressing  
• Personal hygiene 
• Mobility 
• Continence 
• Eating/drinking 
  

b) Occurrence one of the following critical illnesses or/and situations:

• Cancer 
• Coronary artery by-pass surgery 
• Heart Attack (Myocardial Infarction) 
• Kidney failure (End Stage Renal Disease) 
• Stroke 
• Surgeries to aorta 
• Blindness (Loss of Sight) 
• αReplacement of heart valve 
• Major organ transplantation 
• Multiple sclerosis 
• Paralysis 
• Alzheimer disease/Severe dementia (before the age of 61) 
• Angioplasty (PTCA) 
• Benign brain tumor 
• Coma 
• Encephalitis 
• End stage liver disease 
• Loss of limbs 
• Major burns 3rd degree 
• Parkinson’s disease (before the age of 61) 

After the payment of the sum assured according to the benefits (a) and (b), the company will reimburse inside and outside hospital medical expenses and private professional nurse expenses, relating to the covered case, that will occur maximum in five (5) years from the date of the covered case occurrence.
